Physical Examinations

**DO NOT UPLOAD PHYSICAL ON THIS WEBSITE!**

**IT MUST BE GIVEN TO YOUR COACH!**

​Students registering for Athletics must have a current (with in one year) physical examination submitted to coach and then on file in the nurse’s office before the student is authorized to participate in practice/tryouts. To ensure clearance for participation, physicals should be completed and turned in to the coach prior to the first day of practice/tryouts.

RIIL Assumption of Risk Form

Students registering for Athletics at North Providence High School for their first time must submit a RIIL Assumption of Risk Form. This form is found under the above tab "Other Forms".  This form is only required ONCE FOR ALL FOUR YEARS OF HIGH SCHOOL ELLIGIBILITY UNLESS THERE IS A CHANGE IN CONTRACT.

RIPCOA Assumption of Risk Form

Students registering for Athletics at Ricci or Birchwood Middle School must submit a RIPCOA Assumption of Risk Form. This form is found under the above tab "Other Forms".  This form is only required ONCE for all years of Middle School elligibility unless there is a change in contract.
